Derren is supposedly a hypnotist, and I think the whole point of this is that he had hypnotized the people into feeling poorly. Notice how he takes the second guy out of it at the end. Notice what he did with the first guy's hand - when Derren let go, they guy was just holding his hand up and staring blankly at Derren.
